APPLICATION INFORMATION AND INSTRUCTIONS

TYPES OF EVALUATION REPORTS:

Please confirm with the agency or institution that requested you to obtain an evaluation which type of report they require and whether evaluations by private evaluation services are acceptable.

Document-By-Document Evaluation

This report gives the name and description of each diploma or certificate, indicates the period attended, and gives its U. S. educational equivalent. Grade average equivalents will also be provided for reports directed to educational institutions. Suggested for:

Education where transfer credit IS NOT expected
Employment

Immigration

Course-By-Course Evaluation

This report lists all academic studies by courses completed with credits, grades, and grade point average (gpa) in U. S. equivalents.

Suggested for:

Education where advances standing/transfer credit is expected

Professional Licensing or Certification

DOCUMENTATION:

1. Important General Guidelines Which Must Be Followed

a. Clear and legible, original OR certified/notarized (attested) color photocopies of all "original" educational documents must be submitted with each application. All such photocopies become the property of Lisano International.

b. Lisano International reserves the right to request "original" documents for the purposes of verification of authenticity. Such documents will be returned provided that payment for copying ($1 per page) and return postage ($10 priority mail p/h) is received.

c. All foreign language (non-English) documents must be accompanied by a certified English translation. These translations MUST be complete, literal, word-for-word, and in the same format as the original document. Names of degrees, diplomas, and/or certificates should appear in the "native language" and not be translated into U.S. terms such as B.A., M.S., etc.

2. Documentation For Document-by-Document Evaluation Report
If you wish to receive a Document-by-Document Evaluation Report, you must provide official, certified color photocopies of transcripts and all "FINAL" diplomas, degrees, and/or certificates, together with certified English translations (see Item 1c. above) if the original credentials are not issued in English.

3. Request For Additional Documentation
When documentation or information is not complete enough to perform an evaluation, a"REQUEST FOR ADDITIONAL DOCUMENTS" will be sent to you. An evaluation report will be prepared ONLY after ALL supporting documentation has been judged satisfactory by Lisano International.
