APPLICATION INFORMATION AND
TYPES OF EVALUATION REPORTS:
Please confirm with the agency or institution that
requested you to obtain an evaluation which type of report they require and
whether evaluations by private evaluation services are acceptable.
This report gives the name and description of each
diploma or certificate, indicates the period attended, and gives its U. S.
educational equivalent. Grade average equivalents will also be provided for
reports directed to educational institutions. Suggested for:

Guidelines Which Must Be Followed
a. Clear and
legible, original OR certified/notarized (attested) photocopies of all
"original" educational documents must be submitted with each
application. All such photocopies become the property of

wish to receive a Document-by-Document Evaluation Report, you must provide
official, certified photocopies of all "FINAL" diplomas, degrees,
and/or certificates, together with certified English translations (see Item 1c.
above) if the original credentials are not issued in English.

DocumentationWhen documentation or
information is not complete enough to perform an evaluation, a
"REQUEST FOR ADDITIONAL DOCUMENTS"
will be sent to you. An evaluation report will be prepared ONLY after ALL
supporting documentation has been judged satisfactory by
